Occupational medicine coordinators act as liaisons among the injured employee, the Memorial Hermann healthcare providers and the employer. These coordinators are on site at most locations, to provide on-going communication and oversee adherence to your specifications.

This will ensure your preferences are followed when occupational injuries occur after hours or are beyond the scope of smaller clinics. Complete the Employer Protocol Form to engage Occupational Medicine Services for your company. After the form is submitted, a representative will contact your company to review protocol and process your specific service requests.
