By choosing Memorial Hermann Medicare Advantage HMO, you'll be a part of a health plan that's fully connected to your care in Greater Houston. That means you'll get participating physicians, specialists, the Memorial Hermann Health System, and your Medicare Advantage plan all on the same team, uniquely working together for you.

You are eligible to enroll in or switch to a Memorial Hermann Medicare Advantage Plan if you meet the following requirements:

• You are currently enrolled in Original Medicare (Medicare Part A and Part B)
• You live within Harris, Fort Bend, Montgomery, Jefferson, Liberty, or Galveston counties in Texas
• You don’t have disqualifying medical conditions.
• If you have both Medicare and Medicaid coverage and live within Harris, Fort Bend or Montgomery county, you may be eligible for the Memorial Hermann Dual Advantage Special Needs Plan (D-SNP). To learn more, visit the D-SNP section of our website.

Note: There are limited time periods for when you can enroll in a Medicare Advantage plan.
